Healthcare Passport for Patients with Eating Disorders
This document was created in collaboration with Experts by Experience from the EmpowerED Adult Eating Disorders Provider Collaborative. A guidance document has also been created to support both patients and healthcare professionals with the completion of this healthcare passport.
The EmpowerED Healthcare Passport was made to support patients with Eating Disorders (ED) in the following ways:
- Receive appropriate reasonable adjustments
- Avoid triggers to their ED in medical settings
- To ensure a full understanding of co-morbidity between other diagnoses and Eating Disorders
- Improve a patient’s engagement in all aspects of their healthcare
Example Healthcare Passport
This is an example of the passport using a fictional patient.
The passport is designed to be a one-page document, creating a quick snapshot into the most vital needs of the patient. This makes it easier to complete for the person, and to read for the professional.
This card aims to inform professionals that users may have additional needs and might require extra support to enhance their appointment experience. This could involve making them aware of triggers, such as windowless rooms or questions that might provoke a fight-or-flight response.

Onside Youth Zone
Over 40 people came together for the Community Champion Event and watch the ‘Hear my Voice’ video series developed by Level Up, Cheshire and Merseyside, Young People and Families, Lead Provider Collaborative and CANDDID (Centre for Autism, Neurodevelopmental Disorders and Intellectual Disabilities).
‘Hear my Voice’ is a co-produced educational film series featuring:
- Young people from Jigsaw and the team from Cheshire East youth support service
- Young Carers
- Parent Carers/Cheshire East Parent Carer Forum
- CWP Children and Young People Urgent Support Team
- Phil Morris, Director of Specialist Communication Techniques
At the event also saw a series of initiatives launched including:
- Warrington Healthwatch new Awareness Card*, which is designed to empower individuals with lived experience of trauma. This allows health and social care organisations to provide the necessary care without requiring individuals to repeatedly share their stories
- EmpowerED Adult Eating Disorders coproduced Healthcare passport*, aimed at helping people with Eating Disorders in clinical settings receive appropriate reasonable adjustments, avoid triggers to their eating disorders in a medical setting, and ensure a full understanding of co-morbidity between other diagnosis and Eating Disorders.
- Level Up Expert by Experience Bev, launched the Level Up Carers forum film*. The Level Up Parent and Carer Forum is open to all parents and carers whose young people are currently accessing or have previously accessed children and young people’s specialised mental health services.

The event saw speakers from a wide range of organisations including EmpowerED, Adult Eating Disorders, Lead Provider Collaborative North West Experts by Experience Forum, Lead Provider Collaborative Commissioning Team, Level Up, Cheshire and Merseyside, Young People and Families, Lead Provider Collaborative Carers Forum, Helix, Cheshire and Merseyside, Perinatal Mental Health, Lead Provider Collaborative, Experts by Experience Forum, Warrington Healthwatch, Action on Post Partum Psychosis (APP) and Trans without Abuse.

Head of Clinical Commissioning
Duncan Campbell, Head of Clinical Commissioning said: “For us today is important because it's about collaboration.
“We're here with a range of different groups, we've had talks from people in the Lead Provider Collaboratives, including Warrington Healthwatch, Action on Post Partum Psychosis (APP) and Trans without Abuse.
“It’s key for us to connect with different groups of people to collaborate because we know that way we can improve healthcare services for the communities that we serve.”
Ruth, EmpowerED Expert by Experience, who launch the EmpowerED Healthcare Passport said: “I'm here today to present our new healthcare passport that the Forum have created.

Expert by Experience
“It's a one-page profile to give healthcare providers important information about the patients eating disorder and any adjustments they might need.
Jocelyn Ellams, Action on Post Partum Psychosis (APP), Helix Expert by Experience said: “An event like this is really lovely in person and is crucial for networking and ensuring that we’re supporting people effectively.”
Debbie Cannon, Lead from Trans Without Abuse, said: “I'm here today at the Community Champions and Healthwatch Event to raise awareness about Trans Without Abuse.
“I think that the more people that know about my service the better, healthcare settings are quite often the first disclosure for someone who is experiencing in domestic abuse.

Post Partum Psychosis (APP)
“I'm here today to give professionals the tools to support people in the community, empower people to being able to spot signs of domestic abuse, and give people the confidence that they have somewhere they can refer people into a service that is run by a trans person and therefore has an understanding of some of the challenges they face.”
Clare Screeton, Community Outreach Lead, Warrington Healthwatch, said “These events are very important to us as we're meeting all with other members of the community and to share our message as well.
“We do all the feedback for NHS Health and Social Care Services within Warrington, part of our remit is going out and speaking to people, so these type events are very important.
“It's good for us to meet other organisations in the market place that we can tie in with and signpost to.”
